Highly recommended to make documentation thorough! I never took a class nor was one offered, on clinical documentation throughout undergrad and grad school. I felt like my documentation skills were nonexistent. My co-worker introduced me to this book and it’s been incredibly helpful in my documentation especially for biopsychosocial assessments!

The book came in an ok shape. I'd expect for a new textbook for it to come in amazing condition but there's dirt on the edges, the pages are a bit stained, and there's residue on the cover page.

I ordered this book for a class and it came new and wrapped. Part way through my second chapter I discovered there must have been printing problems as there are missing pages and some repeated pages. Would like to receive a new copy of the book.
